In order for your application to be processed and given the appropriate level of priority we require supporting documentary evidence which is detailed on the application form. If this is not given, this will cause a delay in your application being processed and therefore you will not be considered for an allocation.
If you (or someone who lives with you) has a long-term health issue/medical condition/mobility issue or mental health illness that is significant and enduring,
And this is having a substantial or long-term adverse effect on your/their ability to carry out essential day to day tasks;
And your current housing is wholly unsuitable as it is making this health condition / issue worse
And it cannot be made suitable (eg. by being adapted, or by providing the services or help that you/they need);
And so, you (or someone who lives with you) need to move to a property of a specific type (or a specific location) so that you (or someone who lives with you) can do essential day to day tasks
You should complete an Accessible Housing form.
